This role is ideal for an experienced nurse who values thoughtful, patient-centered care and works well independently while staying connected to a supportive team.

Multiple locations throughout Utah County Looking to fill up to 2 weekends a mounth After-hours coverage Friday 5:00 p.m. to Monday 7:55 a.m. Role Overview The On-Call RN provides clinical assessment, guidance, and in-person care during evenings, nights, weekends, and holidays as scheduled. You will serve as a key point of support for patients, families, and care partners outside of regular business hours. Key Responsibilities Respond to after-hours patient needs with in-person visits and through phone triage Assess patients in their place of residence, including private homes, assisted living, and skilled nursing facilities Provide clear guidance and reassurance to patients, families, and facility staff Complete after-hours admissions as needed Document care accurately and in a timely manner Communicate effectively with the interdisciplinary team to support continuity of care Qualifications 1+ year of home health and/or hospice experience preferred Strong clinical assessment and critical-thinking skills Reliable, calm, and professional in urgent or unfamiliar situations Respectful, patient-focused approach to care Detail-oriented with strong follow-through Clear verbal and written communication skills Comfortable using electronic medical records Reliable transportation Licensure & Certifications Active Utah Registered Nurse (RN) license (required) Current CPR certification (required) Valid Utah driver's license with current auto insurance Benefits Flexible scheduling Areas of Care Hospice Home Health Geriatrics Wound Care E04JI802oam74096d28
