PART-TIME PATIENT HEALTH EDUCATOR CG ARIZOLA - CASA GRANDE, AZ 85122
Apply Overview Position Type Per Diem Job Shift Mon 8am-5pm; varying 3 days T-F 8am-12pm Apply Description
STATEMENT OF PURPOSE
To provide support for the Clinical Pharmacy and Diabetes Education Departments, and to aid in effective and efficient patient communication. Provide ongoing management support to the CDCES and other members of the department. Provide ongoing support to clinical staff, including completing prior authorizations for medical staff.
ESSENTIAL FUNCTIONS
Referrals Addressing Provider Referrals from EHR and patient self-referrals. Chart and Teams QI entry of Necessary data required for continuation of Diabetes Accreditation. Follow standing orders regarding lab orders, DEE and Dental referral. Scheduling group and individual sessions. Prior Authorization Support Complete medication prior authorizations for clinical staff, under direction of clinical pharmacy department Assist in filling out, sending, and following up on patient assistance foundation programs. Supplies Order educational material (BD, Novo, and other on line material). Order free insulin kits, syringes, and needles. Order glucometers, home blood pressure cuffs, and office supplies. Remind providers to order medication samples. Education Able to be trained to provide group and individual education on the following: At home blood pressure monitor set up Hypoglycemia signs and symptoms and treatment of Glucometer training to include BG monitoring, BG logging and BG goals CGM and Sensor training Training of injectable DM medication Downloading of glucose Meters, CGM, and Insulin Pumps Diabetes electronic apps, as indicated Insulin Administration including Vial and Pens via provider's orders Able to document services rendered correctly. Review new material including attending pertinent educational classes for personal/department growth and assisting with providing published and up to date curriculum applicable to target population. Vaccines and A1C for Referred DM patients Administer vaccines as due, via diabetes standing orders Draw in house A1C, BG as indicated, via standing orders Insulin Sampling Take Inventory of insulin and other injected medication samples. Communicate with Drug Representatives and Providers regarding online medication sampling orders as needed. Proper use of Dymo Label maker for proper labeling of medication samples and forms. Assist in Patient Preparation for Clinical Pharm/Diabetes patient visits Take patient vitals and weight. Download Glucose Meter, CGM, Insulin Pump as indicated. Attain copy of BG log if indicated Document NO SHOW on EHR and contact patients via mail or patient phone calls for rescheduling missed appointments. Preparation of educational material needed in sessions. Cleaning and setting up Consult rooms / classroom as needed. Keep Education Material supplied. Keep Information Board or Consult rooms up to date Schedule classes and or meetings via Public Drive Calendar.
DEAP QI / OTHER
Attain ZARA reports as indicated. 2. Data entry of Pre-Program A1C and 3-month post follow up. . Quarterly Reports of QI data attained for evaluation and improvement. Assist with DEAP current Accreditation cycle and Standards D aily Secretary Activities Answering e-mail and phone messages. Keep office organized and neat. Keep daily log of Refrigerator temperature twice daily. Copying reports and sending to the appropriate provider/company/or individual.
ADDITIONAL RESPONSIBILITIES
Participate in-service/education regarding Quality Improvement. Perform all other related duties as assigned or requested. Qualifications
EDUCATION AND EXPERIENCE
High School diploma or equivalency Completion of approved medical assistant program or pharmacy technician program desired , medical assistant certification or pharmacy technician certification preferred Diabetes prevention assistant certification desired Share job details to
