Job Description:
  • 7:00 PM to 7:00 AM, totaling 36 hours per week, with every other weekend required Coordinates referrals from interdisciplinary team members and facilitates patient care management across the continuum.
Ensures quality patient care and regulatory compliance. Participates in daily multidisciplinary rounds to review the progression of care and discharge plans for all assigned patients. Remains at the bedside of assigned patients at all times unless relieved by an appropriate staff member. Assists in calming and reorienting patients in cases of agitation or confusion. Removes visible hazards and broken equipment from the immediate environment and reports as appropriate. Records all observations accurately and promptly. Provides support to patients with complex psychosocial needs under the guidance of a Care Management mentor. Performs other duties as assigned.
