What You'll Do:

Process internal and external referrals while adhering to established guidelines and policies Assist patients in locating healthcare and community resources Schedule follow-up appointments and coordinate care after hospitalizations Conduct 30-day referral follow-up calls and document outcomes appropriately Ensure referrals are properly completed and closed out Answer patient questions by phone and in person Contact patients from ER referral and discharge lists to offer follow-up appointments at CHW Assist with patient follow-up reporting and documentation Support CIHCP referral coordination and documentation processes as assigned Maintain accurate documentation while adhering to HIPAA and organizational policies Coordinate with community partners and other departments as needed Respond to public health emergencies and support organizational initiatives Carry out the mission, vision, and values of the organization Perform other duties as assigned

What We're Looking For:

High school diploma or equivalent 1 year of experience providing comprehensive case management or social services to underserved populations Experience working with underserved populations in healthcare, social services, or case management settings Knowledge of Galveston County community resources and public assistance programs Familiarity with medical terminology and case management processes Strong customer service, communication, and computer skills (MS Office) Ability to work independently and manage multiple priorities Ability to work evenings and weekends as needed Valid Texas driver's license (or ability to obtain) and reliable transportation Must comply with GCHD immunization policy and ICS training requirements Must pass a criminal background check and drug/alcohol screening

Preferred Qualifications:

Bilinugal in Spanish Experience coordinating referrals and follow-up care
