Job Description

Overview We are seeking a dedicated and knowledgeable Care Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the appropriate medical services and support throughout their treatment journey. This position requires a strong understanding of medical terminology, acute care processes, and effective utilization management strategies. The role would also include providing telehealth support for our providers at long-term care facilities. Responsibilities Collaborate with multidisciplinary teams to coordinate inpatient care and ensure seamless transitions between different levels of care. Monitor patient progress and adjust care plans as necessary, utilizing knowledge of physiology to inform decision-making. Provide education and support to patients and their families regarding treatment options and health management strategies. Implement utilization management practices to optimize resource use while maintaining high-quality patient care. Maintain accurate documentation of patient interactions, care plans, and progress notes in compliance with medical standards. Stay informed about current best practices in acute care and infectious disease management to enhance patient outcomes. Must be willing to travel to various nursing homes in the service area to support provider telehealth visits. Qualifications Medical Assistant or Certified nursing assistant required; experience in acute care settings preferred. Strong knowledge of medical terminology and physiology. Experience with diagnostic evaluations and inpatient care management is highly desirable. Familiarity with utilization management principles and practices. Ability to work collaboratively within a multidisciplinary team environment. Excellent communication skills, both verbal and written, for effective patient education and team coordination.
