Key Responsibilities:

Conduct comprehensive annual home health risk assessments in patients' residences Review prior and current medical history, chronic conditions, and medication regimens Educate patients on managing their health conditions and proper medication use Identify potential gaps in care, offering recommendations to improve patient outcomes Visits may include annual health assessments or post-discharge evaluations No prescribing medications or developing treatment plans

Qualifications:

Active Nurse Practitioner license in Oregon Board Certification through AANP or ANCC Current CPR (BLS) certification 6+ months of current nurse practitioner experience preferred MSN degree from an accredited nursing program This opportunity provides competitive per-assessment pay between $80 and $90, plus a mileage stipend and compensation for administrative time.
