Job Description

Nursing degree, active Nurse Practitioner (NP) license and minimum 1+ years Home Health Nurse Practitioner (NP) experience required. Applicants who do not meet these qualifications will not be considered. Bring your expertise as an experienced Nurse Practitioner to impactful community-based care by conducting in-home health risk assessments for Medicare Advantage members. This per diem opportunity offers flexibility and autonomy, covering Jamestown and locations throughout Chautauqua County. Design your own schedule and help improve patient outcomes while working closely with diverse populations.
Schedule:
Flexible shifts available Monday through Sunday Self-determined schedule—choose your own days and hours Minimum commitment on a weekly or bi-weekly basis
Key Responsibilities:
Perform comprehensive home health risk assessments averaging 45 minutes per visit Evaluate patient medical histories, current health status, and medication regimens Educate patients on their medical conditions and prescribed medications Identify gaps in care and make actionable recommendations for follow-up All visits are assessment-only; no treatment planning or prescribing is necessary Assignments may occur anywhere within Chautauqua County, including towns such as
Dunkirk, Mayville, and Chautauqua Qualifications:
Active Nurse Practitioner (NP) license in New York MSN from an accredited nursing program Board Certified via AANP or ANCC Current CPR/BLS certification Six months or more of recent NP experience preferred Adaptability to travel within Chautauqua County for scheduled visits
