Monday - Friday 8:00AM-4:00 PM (40 hr/week) ability to be flexible with schedule is highly preferredEssential Job Functions:
- The Person-Centered Plan Manager, also known as a case manager, works directly with the individual in the community to help them achieve their goals. Typically, approximately 70% of time is spent in the community.
- Connects the individual with the services they need to maintain and expand their independence. This may include community services, government-funded benefits, or other publicly available resources.
- Advocates for the individual ensuring that they receive the proper support for their situation.
- Completes administrative tasks on behalf of the individual, including consultations, documentation into an electronic record system, benefits eligible forms, and other related tasks.
Qualifications:
- Associate degree or equivalent experience, preferably in a related field.
- Valid driver's license with an acceptable driving record and ability to pass background checks.
- Basic computer skills, including Microsoft Office, email communication, internet, and documentation entry.
- Ability to work independently and collaboratively as part of a team.
- Compassion, resourceful, and a strong desire to learn and grow.
