PHARMACIST JOB DESCRIPTION
Positions 1. Registered Pharmacist - FULL TIME (Day Shift) Job Summary The Staff Pharmacist supports safe, effective medication therapy in an acute inpatient psychiatric hospital by reviewing and verifying medication orders prior to dispensing and administration. The pharmacist evaluates each order against the patient profile (indication, dose, allergies, interactions, labs, and duplications) and collaborates with prescribers and nursing to resolve issues. This role also supports daily medication distribution, regulatory compliance, safe handling and documentation of controlled substances and other regulated medications. Key Responsibilities (
Note:
this is representative, not exhaustive) Review and verify inpatient medication orders prior to dispensing; assess appropriateness for psychiatric and medical comorbidities. Identify and resolve medication-related problems (allergies, interactions, contraindications, duplications, dosing, and monitoring needs). Communicate with prescribers and nursing to clarify orders and recommend alternatives as clinically appropriate. Provide drug information and clinical consultation to prescribers, nurses, and other staff. Dispense medications in accordance with hospital policy and state and federal regulatory requirements. Support medication reconciliation processes assigned. Provide direct oversight and final check of work performed by technicians and other non-licensed personnel (for example: unit-dose fills, cassette/cart fills). Maintain accurate documentation and records for controlled substances, investigational medications (if applicable), recalls and returns, and other required logs. Support automated dispensing cabinet workflow (Pyxis) including restocking and verification processes, discrepancy resolution, controlled substance oversight, and routine station reviews as assigned. Participate in quality and safety activities, including clinical interventions documentation, audits, competencies, and process improvement work. Perform other related duties as assigned to support pharmacy operations. Monitoring/Following patients on High-Risk High Alert Medications, Clozapine, Warfarin, Opiates. Antibiotic Stewardship & Opiate Stewardship. Other QAPI activities. Minimum Qualifications Graduate of an Accredited College of Pharmacy. Current New York State Board of Pharmacy License in good standing. Strong communication, professionalism, and documentation skills. Preferred Qualifications Hospital experience and/or high-volume retail experience is a plus. Pyxis or automated dispensing cabinet experience is preferred but not required. Experience with inpatient behavioral health or psychiatry is a plus but not required.
