Flexible working hours Safe work environment We are seeking a Physical Therapist to join our growing team at Senior Outpatient Services, LLC. A Physical Therapist will be involved with providing treatment to older adults recovering from serious injuries, assisting in their functional and motor development, and helping them achieve their personal goals in a safety and comfort of their own home. The ideal candidate will have an understanding of progressive patient treatment, will be creative in developing new techniques to improve patient health, and feel passionate about helping our older adult population remain safe in their homes.
Responsibilities:
~Review available patient information and evaluate the impact of this information on the assessment and treatment process. ~Conducts and documents a thorough functional assessment of the patient utilizing standardized procedures. ~Develop and implement an individualized treatment plan based on patient needs and containing achievable functional goals. ~Comply with evaluation, treatment and documentation guidelines. ~Orient patients to program services and treatment procedures appropriate to their needs. Inform patients of any potential risk during any procedure. ~Work with patient, caregivers, and families to maximize patient's use of functional abilities. ~Establish and instruct patient and family or caregivers in exercises and therapeutic procedures to be continued following discharge. ~Assist in the referral process when a patient requires additional services following discharge. Maintain current knowledge of community resources. ~Access continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical competency.
