Eddy SeniorCare PACE (Program of All-Inclusive Care for the Elderly) is an exciting, innovative model of care providing comprehensive medical and supportive services to older adults who would otherwise need nursing home care. Our beautiful 83 acre hillside campus at 385 Watervliet-Shaker Road, Latham is looking for an experienced Physical Therapist who wants to make an Impact to join it's team of Rehab Therapists. Full-Time Physical Therapist - Eddy SeniorCare PACE (Latham, NY) Make a difference where it matters most. We are seeking a compassionate and driven Physical Therapist to join our interdisciplinary team in Latham. This is more than a therapy role—it's an opportunity to build relationships, influence care plans, and see the long-term impact of your work every day. Mission-driven work serving frail elders in the community Interdisciplinary care model - collaborate with physicians, nurses, social workers, and more Meaningful relationships with participants (not volume-driven care) Autonomy & variety - clinic, home visits, and team-based planning Supportive culture rooted in respect, dignity, and compassion What you will do: As a Physical Therapist, you will play a key role in improving function, independence, and quality of life for our participants Conduct comprehensive PT evaluations and develop individualized care plans Provide hands-on therapeutic interventions and monitor progress Collaborate with the interdisciplinary team to coordinate holistic care Perform home assessments and recommend adaptive equipment and DME Educate participants, families, and caregivers on mobility, safety, and exercise programs Support discharge planning and ongoing functional independenceWork Schedule:
Monday-Friday, generally 8am-4:30pm What you will need: Licensed Physical Therapist in New York State At least 2 years of PT experience required, ideally with geriatrics and/or home care Strong clinical skills and passion for working with older adults Excellent communication and team collaboration skills Ability to travel locally for home visits What Makes This Role Unique At Eddy SeniorCare PACE, you'll: Focus on function over volume—no rushed back-to-back outpatient schedule Be part of a true interdisciplinary model, not siloed care Work with participants longitudinally, seeing their progress over time Have the chance to shape care plans and influence outcomes directly Ready to Make an Impact? $37.60-$48.10 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location."
