$100,000 to $120,000 per year plus $10,000 Sign-On BonusJob OverviewA growing home health organization is seeking Physical Therapists (PTs) to join its Skilled Home Health team serving patients throughout Hillsdale, Lenawee, and Monroe counties in Michigan.

In this role, the Physical Therapist will provide in-home therapy services focused on helping patients restore mobility, improve strength, and regain independence following illness, injury, or surgery. The PT will complete evaluations and start-of-care visits while collaborating closely with interdisciplinary care teams to develop individualized treatment plans that support optimal patient outcomes.

Compensation & IncentivesBase Salary:
$100,000 to $120,000 per year$10,000 Sign-On BonusProductivity Bonus StructureExpected productivity: 27 points per weekAdditional compensation of $65
  • 75 per point for productivity above targetAnnual bonus opportunitiesCompany vehicle available for therapists driving 1,400+ miles per monthKey ResponsibilitiesProvide physical therapy services to patients in their homes in accordance with physician orders and care plansPerform patient evaluations and start-of-care visitsDevelop and implement individualized therapy treatment plansEducate patients and caregivers on exercises, mobility techniques, and safety strategiesDocument patient progress and treatment outcomes in the EMR systemCollaborate with nurses, physicians, and interdisciplinary care teamsComplete required clinical documentation including OASIS assessments when applicableClinical Support & WorkflowPTs focus primarily on evaluations and start-of-care visitsPhysical Therapist Assistants (PTAs) assist with follow-up visitsDedicated scheduling team manages patient visit schedulingEMR system: Homecare Homebase (HCHB)QualificationsLicensed Physical Therapist (PT)Current Michigan PT license requiredHome health experience preferredOASIS documentation experience preferredExperience with Homecare Homebase (HCHB) preferredStrong clinical assessment and patient education skillsAbility to work independently while managing a patient caseloadOrientation & TrainingNew hires complete a two-day branch orientation followed by field precepting, with training length based on experience.
