We're eager to welcome dedicated home health Physical Therapists to our team! Pay is competitive and commensurate with experience. New grads are welcome as well as professionals without prior home health experience. Training is provided to therapists new to home health. Our electronic documentation is extremely easy. See as many or as few patients as you want! We utilize TherapySync for documentation, which is quick and easy to use. We are currently hiring for the following areas: Redlands, San Bernardino, Yucaipa, Banning, Menifee, Murrieta, Riverside, Fontana, Corona, Moreno Valley, Calimesa, Colton, Hemet, Hesperia, Victorville, Beaumont, Cherry Valley, Rialto, and all surrounding areas. Responsibilities Make home visits to clients in the community, as well as Senior Living, Assisted Living, and Independent Living facilities. Assess cognitive, psychological and motor aspects of performance and develop individualized intervention plans to achieve favorable outcomes. Evaluate home environment for hazards or barriers to more independent living. Identify equipment needs and environmental adaptations resulting in enhanced outcomes for clients. Continually assess and revise the physical therapy care plan, and participate with nursing in the multidisciplinary care plan, as appropriate. Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety and independent living. Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided. Assess each patient and develop an appropriate treatment plan with measurable goals and time frames Prepare and submit clinical and progress summaries based on identified and attained goals Ensure that other treating medical professionals have the necessary information to appropriately evaluate the patient Qualifications Doctor of Physical Therapy degree of an accredited physical therapy curriculum Certification in Physical Therapy and valid state license to practice Active drivers license, registration, and insurance CPR New grads are welcome to apply, training is provided!
