Duties Assess patients' physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to develop tailored care plans. Implement evidence-based physiotherapy techniques such as trigger point therapy, orthopedics, and geriatrics interventions to promote healing and mobility. Coordinate discharge planning with healthcare teams to ensure smooth transitions from hospital or inpatient settings to home care. Deliver patient-centered therapy sessions across outpatient clinics, hospital environments, and community settings, including pediatrics when applicable. Educate patients and their families on proper exercise routines, injury prevention strategies, and self-care techniques to foster independence. Maintain accurate documentation of patient progress using electronic medical records (EMR) systems while adhering to medical terminology standards. Participate in interdisciplinary team meetings to review care plans and adjust treatment strategies based on patient response and evolving needs. We're looking for a Physical Therapist to join a Home Health team in beautiful Northern California—where your patients are grateful, your schedule is yours, and your weekends are actually off. The Role 8-hour days, Monday-Friday 40 guaranteed hours per week ~5 visits per day (~40 miles/day, reducing as the team grows) Service area: most of Tehama County Patient mix: orthopedic and joint replacements, geriatric patients with CHF, COPD, and multiple comorbidities PTs handle SOC visits on ortho cases when nursing isn't ordered
