Per diem Join our team at Allegria Village , where we are committed to providing exceptional, compassionate care to our residents. We are currently seeking a dedicated and motivated Per Diem/Contingent Physical Therapist to join our Skilled Nursing Facility team. Key Responsibilities Provide direct patient care and perform thorough functional evaluations to develop individualized treatment plans Design, implement, and adjust plans of care to achieve optimal patient outcomes, incorporating family participation when appropriate Ensure all therapy practices align with established standards of care and facility policies Supervise and guide support staff to help patients meet their functional goals Collaborate effectively with the interdisciplinary care team to promote positive outcomes Maintain compliance with facility and departmental policies, including quality improvement, safety, and infection control protocols Complete and maintain accurate documentation, including evaluations, goals, plans, and physician prescriptions Participate in continuing education, workshops, and professional development activities to enhance skills and stay current with best practices Foster positive relationships with patients, families, and colleagues to ensure a supportive care environment Perform other related duties as assigned Education & Certification Requirements Graduate of an accredited Bachelor's, Master's, or Doctorate program in Physical Therapy Current license in good standing with the State Board of Physical Therapy Examiners Qualifications Knowledge and skills specific to the Skilled Nursing Facility (SNF) population Previous SNF experience preferred Understanding of all applicable state, federal, and professional standards Strong problem-solving, organizational, and communication skills (both verbal and written) Demonstrated compassion, empathy, and professionalism when working with residents and families Excellent customer service orientation and ability to work collaboratively in a team environment Pleasant, positive demeanor and a commitment to delivering quality care Physical Requirements The physical demands described are representative of those necessary to perform the essential functions of this role. Frequent walking, standing, pushing, pulling, reaching, crouching, stooping, bending, and stretching Ability to transfer patients safely and perform required documentation and computer tasks
