26 points per week

Part-Time:

15 points per week Point Structure SOC

• 1.5 points | Recertification
• 1.5 points | Routine Visit
• 1 point What You'll Do Evaluate patients and develop individualized treatment plans Help patients improve mobility, strength, and independence Provide education to patients and caregivers Collaborate with physicians and the care team Document visits in Axxess EMR Requirements Active Illinois Physical Therapist license Reliable transportation and valid driver's license Home health experience preferred but not required Ability to attend orientation and training in Elgin, IL Join a growing team that values clinicians and supports exceptional patient care.
