Primary Responsibilities:

Complete in-home assessments focused on functional mobility and strength Develop and implement personalized rehabilitation plans Provide hands-on therapy, exercise progression, and equipment recommendations Communicate effectively with nurses, aides, providers, and caregivers Document treatment progress and adapt plans to maximize outcomes

Requirements:

Current Minnesota Physical Therapist license Valid driver's license and reliable vehicle Home health experience preferred, but outpatient or inpatient PTs are welcome Genuine passion for patient-centered care and community-based rehabilitation
