Key Responsibilities:

Perform assessments, follow-up visits, and chronic disease management. Provide primary care and community rounding services. Collaborate with caregivers and interdisciplinary teams. Build strong relationships with patients and families. Support continuity of care and care coordination initiatives.

Qualifications:

Graduate of accredited Physician Assistant program. Active Minnesota PA license required. Experience in primary care, hospital medicine, geriatrics, family practice, emergency medicine, or senior care preferred.
