Requirements:

Board Certification :

Board Eligible or Certified in

Hospice and Palliative Medicine Licensure :

Valid Washington State license or eligibility under the Interstate Medical Licensure Compact (IMLC)

Certifications :

No specific BLS, ACLS, or PALS certifications required
