This provider will deliver comprehensive, patient-centered care to individuals across the lifespan, with a focus on preventative medicine, chronic disease management, and health promotion. Key Responsibilities Provide primary care services including assessment, diagnosis, and treatment of acute and chronic conditions Perform routine physical exams and wellness visits Manage chronic diseases such as diabetes, hypertension, and asthma Order and interpret diagnostic tests and labs Prescribe medications in accordance with the NYS regulations Collaborate with physicians and interdisciplinary team members Educate patients and families on health maintenance and disease prevention Maintain accurate and timely documentation in the electronic medical record Qualifications Graduate of an accredited NP or PA program Current NYS license Board certification (FNP, ANP, or PA-C) DEA certification (or Eligibility) Prior primary care experience preferred, but new graduates may be considered Medent EMR experience is a plus Schedule Monday -Thursday One evening a week 13 Saturdays a year 13 on-call weeks or weekends a year(coordinated with your Saturday) Compensation Based on experience; ranges from $56 to $65 per hour Benefits Four weeks of vacation One week of CME time off CME money based on the number of hours worked 401k with 5% match after one year Paid malpractice insurance Health insurance with cost share based on number of hours worked Mission Statement "Where we provide cheerful, compassionate, competent healthcare so as to openly glorify God."
