Job Overview Gratitude Adjustment Integrated Healthcare is seeking an experienced Physician (MD/DO), Physician Assistant (PA) or Nurse Practitioner (NP) to join our growing team in Nancy, Kentucky . This provider will offer primary care family medicine services within an integrated healthcare setting in a rural community that includes behavioral health, medication-assisted treatment, and holistic wellness services. We are looking for a compassionate and experienced provider who values patient-centered care, collaboration, and community impact . This is an excellent opportunity for a provider seeking part-time work in a supportive, multidisciplinary environment . Duties Provide comprehensive family medicine services to patients of all ages Conduct physical exams, diagnoses, and treatment planning Prescribe medications and manage ongoing care Collaborate with behavioral health providers and care team members Maintain accurate and timely documentation within the EMR Participate in integrated care coordination when appropriate

Qualifications:

MD/DO, Physician Assistant (PA) or Nurse Practitioner (NP) licensed in Kentucky Board Certified or Board Eligible (for MD/DO) Active DEA license Prior family medicine or primary care experience required Strong communication and patient engagement skills Ability to work independently in a collaborative environment

Preferred Experience:

Integrated care or behavioral health settings Medication-assisted treatment (MAT) Rural healthcare experience
