This position is PRN. The teammate will work as needed. Pay Range $41.10- $61.
Major Responsibilities:
Patient Assessments / Reassessments Performs appropriate patient assessments resulting in an effective treatment plan. Reassesses patients at appropriate intervals of time demonstrating the ability to observe treatment effect and modify the plan of care as indicated. Involves the patient and /or family in the assessment process and the formation of the care plan. Care Planning / Interdisciplinary Communication Provides care in accordance with physician's orders. Demonstrates appropriate case management to ensure quality of care and best patient outcome. Communicates appropriately to physicians, nurses, case managers, and other members of the health care team to ensure care is collaborative and appropriate. Patient Outcomes Assesses and documents patient outcomes criteria at the initial and discharge evaluations. (all shoulder, back, incontinence, and anodyne). Physical Therapy Modalities / Patient Treatment Demonstrates knowledge of physical therapy modalities / treatments and performs correctly. Operates Equipment Safely and Correctly Demonstrates knowledge of and competency in using equipment pertinent to providing physical therapy services. Patient Education Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning. Includes the family in teaching as appropriate. Documentation Documentation meets current standards and policies. Documentation is legible. No inappropriate abbreviations are used. Documentation is completed in a timely manner. Medical Records are closed as per policy. Charge Capture Time is adequately documented in the assessments and daily treatment notes. Each charge bill adequately reflects all services provided. Demonstrates self-growth as a professional therapist. Engages in continuing education and professional growth opportunities Licensure, Registration, and/orCertification Required:

Good interpersonal and communication skills, time management skills, and the ability to be flexible and dependablePhysical Requirements and Working Conditions:
Must be able to use visual near and distance acuity. Must have verbal ability to communicate in person and over the phone or radio. Must be able to assist co-workers in transfers and transport supplies, patients/residents, or equipment using good body mechanics. Must have functional range of motion of the cervical, thoracic and lumbar spines, upper and lower extremities with a grip strength of 60-80 pounds, specific to the job class evaluation. Must be able to forward reach, overhead reach, bend, squat, kneel and change from one position to the other rapidly. Must be able to lift 25-60 lbs. and carry 25-50 pounds specific to job class evaluation. Must be able to tolerate high noise, busy atmospheres and noxious odors. Must be able to work in noisy environments and be self directed to complete jobs as ordered. Must be able to remain calm under stressful situations. Must be able to assess and respond to multiple situations simultaneously. Must be able to assist co-workers in multiple overlapping tasks simultaneously. Must be able to prioritize job tasks and organize work space to be efficient and conducive to completing work tasks as assigned in a timely manner. The physical therapist is responsible for patient evaluations, care planning, directing, and administering physical therapy modalities of treatment as prescribed by a licensed physician.
