Job Description

About the Alaska Primary Care Association (APCA) The APCA is a non-profit association that was established in 1995. Our mission is Advancing Alaska's Wellness. We work to support the development and operation of Alaska's 29 federally qualified health centers and look-alike organizations.
What this role is:
The Program Manager - Nutrition, Chronic Disease, and Oral Health/Quality Improvement is a dynamic role focused on strengthening Alaska's Community Health Centers by advancing whole-person care through nutrition, chronic disease management, oral health integration, and quality improvement. Embedded within the Health Center Operations & Quality Department, this position blends strategic program development with hands-on technical assistance to help health centers build sustainable, effective systems of care. You'll play a key role in aligning clinical priorities with operational excellence, ensuring that health centers have the tools, workflows, and support needed to improve outcomes across the care continuum.
What You'll Do:
Provide/coordinate technical assistance and training on nutrition, chronic disease management, oral health integration, and quality improvement Support health centers in identifying workflow gaps and implementing tailored, sustainable improvements Guide implementation of screenings, interventions, and Plan-Do-Study-Act (PDSA) cycles Develop tools, resources, and best practices to strengthen clinical and operational performance Collaborate across departments and with partners to deliver coordinated, high-quality support Collect, analyze, and apply data to measure program impact and inform continuous improvement Contribute to program development, workplans, and performance tracking aligned with health center needs Participate in site visits, peer learning opportunities, and workforce-focused initiatives Manage partnerships and maintain strong relationships with stakeholders and subject matter experts Support reporting, compliance, and alignment with funding and organizational priorities
