Psychiatric Mental Health Nurse Practitioner (PMHNP) Hometown Health Center (HHC) is seeking a compassionate and experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our integrated care team. This position offers a unique opportunity to provide critical behavioral health services to individuals across the lifespan in a supportive, community-based setting. Full-time or part-time options are available. About Hometown Health Center Established in 2003, HHC is a Federally Qualified Health Center (FQHC) that provides medical, dental and behavioral health services to over 8,200 patients annually across Central Maine. As a leader in community-based care, we offer more than 32,000 visits per year and are proud to serve individuals and families who may otherwise face barriers to care. With locations in Dexter, Palmyra and a School-Based Health Center in Newport, HHC continues to expand. Our team is mission-driven, collaborative and committed to building healthier communities. About the Role The Psychiatric Mental Health Nurse Practitioner (PMHNP) provides direct psychiatric care, including evaluations, medication management and ongoing treatment to individuals with a wide range of mental health conditions. Working as part of a multi-disciplinary team, the PMHNP plays a vital role in supporting patients' overall well-being through integrated care. Key Responsibilities Conduct psychiatric evaluations and mental health assessments Develop individualized treatment and medication plans Prescribe and manage psychotropic medications in accordance with current best practices Collaborate with behavioral health providers, primary care clinicians and support staff to ensure coordinated care Monitor treatment response and adjust care plans as needed Maintain accurate, timely and confidential documentation Provide crisis intervention and consultation as needed Minimum Qualifications Certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) Current Nurse Practitioner license in the State of Maine Active DEA license with authority to prescribe Schedule II-V substances CPR certification (or willingness to obtain) Preferred Qualifications Experience in a community health or outpatient mental health setting Comfortable treating patients across the lifespan (children, adolescents, adults) Strong communication and interpersonal skills Commitment to trauma-informed, patient-centered and culturally competent care
