Job Description

Overview The Part-Time In-Home Supports LPN will serve as a crucial member of our interdisciplinary team, ensuring compliance for in-home support cases. This role provides support to families as needed, including but not limited to conducting required trainings, reviewing and evaluating individual service plans, and maintaining/auditing documentation in compliance with agency, regulatory, DDD & Medicaid requirements. This position reports to the Chief Operations Officer.
Essential Job Functions:
Ensure Individual Service Plans (ISP's), medical information & scripts/trainings are up to date. Monthly Review of Medication Administration Records (MARS) and over-the- counter (OTC) forms/documentation. Ensure all required medical documentation is on file, up to date and accurate Ensure that any required prescription information is documented, on file and up to date Case Management Training and Instruction Medical Support Conduct training for agency employees as assigned and required by DDD and agency policy. Works with Training Specialist and the Quality Department to ensure compliance. Complete all required corporate, NJ College of Direct Support (CDS), and annual trainings in a timely manner. Conduct training to in-home DSP's on individual specific needs - adaptive equipment, special diet, etc.
  • Member of the New Hire Orientation Training process for in-home supports.
  • Provide instruction and complete oversight of the Medication Administration Curriculum for new employees, ensuring thorough training in medication administration protocols and procedures.
  • Conduct annual re-certification sessions for current employees to maintain competency in medication administration practices and compliance with regulatory requirements.
  • Responsible for conducting a quarterly family nursing meeting with families.
Documentation and Compliance:
  • Keep updated with changes in the nursing field and maintain all required documentation in compliance with company procedures and regulatory requirements.
  • Fully participate in internal or NJ State investigations or audits
  • Ensures all training is documented and sent to Training Specialist for employee files
Committee Membership:
  • Serve as a member of the Behavior Management Committee (BMC) and Human Rights Committee (HRC), actively participating in discussions and decision-making processes.
  • Attend all meetings as assigned, providing input and expertise regarding healthcare and nursing considerations.
Individual Support Plans and Person-Centered Planning:
  • Develop and update Individual Support Plans (ISP) and Person-Centered Plans in collaboration with an interdisciplinary team.
  • Incorporate concerns, observations, and behavioral development considerations into Individual Support Plans (ISP) and Person-Centered Planning processes to ensure holistic care and support for individuals served.
  • Member of the ISP Team.
Minimum Qualifications:
Licensed and currently registered as a Registered Nurse in the State of New Jersey. Graduate of an accredited program of professional nursing. At least one year of experience as a Licensed Practical Nurse in delivering community-based health services to individuals with developmental disabilities. Valid driver's license with good standing and maintain active vehicle insurance. CPR Certification. Knowledge of standard OSHA and Safety Data Sheets (SDS) safety procedures. Ability to read, write, speak, and understand English. Excellent communication skills, both written and verbal. Good organizational and interpersonal skills. Ability to maintain strict confidentiality. Knowledge of equipment used by persons receiving services. Willingness to work travel to in-home cases. Successful clearance of all pre-employment requirements including background checks, fingerprinting, CARI, and drug screening.
  • Completion of agency Medical/ Addressing Enhanced Needs (AEN) forms, Nursing Notes.
