Job Description
Role Overview This is a full-time, on-site QIDP (Qualified Intellectual Disabilities Professional) position responsible for overseeing the development and implementation of individualized care plans, service coordination, and ongoing support for individuals with intellectual and developmental disabilities. The QIDP will work closely with individuals, families, guardians, interdisciplinary teams, staff, and state agencies to ensure high-quality, compliant, and person-centered services. Essential Duties and Responsibilities Provide ongoing assessments, advocacy, case planning, financial planning, service coordination, and monitoring of services for a caseload of individuals with IDD. Develop, implement, and monitor individualized care and service plans. Train and support staff in implementing care plans and interventions. Maintain compliance with Medicaid and state regulations for ICF/IDD programs, including accurate record-keeping and documentation. Collaborate with interdisciplinary teams, service coordinators, families, guardians, and state surveyors. Advocate for individuals with IDD to help them achieve personal goals and maximize independence. Safely assess and address medical and behavioral needs, recognizing and reporting concerns promptly. Adapt to changing priorities while managing multiple details effectively. Minimum Qualifications Bachelor's Degree in Human Services, Psychology, Social Work, or a related field. Strong verbal and written communication skills to interact effectively with clients, staff, families, guardians, service coordinators, and state surveyors. Understanding of Medicaid and state regulations governing ICF/IDD programs. Ability to develop, monitor, and adjust individualized care plans. Excellent organizational, time management, and problem-solving skills with strong attention to detail. Proficiency in Microsoft Office applications. Ability to work independently (self-directed) while functioning as part of a team. Empathy, compassion, and a genuine commitment to promoting dignity and independence for individuals with IDD.
