Job Description

Position Summary We are seeking a compassionate and detail-oriented Registered Nurse (RN) to oversee patient care and supervise homemakers providing services under the West Virginia Medicaid Waiver and Personal Care programs. The RN will ensure high-quality, compliant, and person-centered care for individuals receiving in-home services. This role combines clinical oversight, care coordination, staff supervision, and regulatory compliance responsibilities. Key ResponsibilitiesClinical Oversight & Patient Care Conduct initial and ongoing nursing assessments for patients enrolled in Waiver and Personal Care programs. Develop, implement, and update individualized care plans in accordance with physician orders and state regulations. Monitor patient health status and ensure services align with assessed needs. Provide clinical guidance to homemakers and personal care staff. Identify changes in condition and coordinate appropriate interventions. Maintain accurate and timely clinical documentation. Staff Supervision & Support Supervise homemakers and personal care aides to ensure quality service delivery. Conduct competency evaluations and performance reviews. Provide training and ongoing education related to patient care, safety, and compliance. Serve as a clinical resource for field staff. Care Coordination Collaborate with physicians, case managers, families, and interdisciplinary team members. Participate in care conferences as required. Ensure communication between agency staff and other healthcare providers. Compliance & Quality Assurance Ensure adherence to West Virginia Medicaid Waiver and Personal Care program regulations. Maintain compliance with state and federal healthcare guidelines. Participate in quality assurance activities and audits. Report incidents and ensure proper documentation and follow-up. Qualifications Active and unrestricted Registered Nurse (RN) license in the State of West Virginia. Graduate of an accredited nursing program. Minimum of 1-2 years of clinical nursing experience (home health or community-based care preferred). Knowledge of WV Medicaid Waiver and Personal Care regulations preferred. Strong assessment, organizational, and communication skills. Ability to supervise and mentor non-clinical staff. Valid driver's license and reliable transportation. Ability to travel within assigned service areas. Physical Requirements Ability to travel to patient homes. Ability to perform nursing assessments in various home environments. Ability to lift up to 25 pounds as needed. Work Environment Combination of office and in-home patient settings. Flexible scheduling may be required to meet patient needs.
