Job Description

Position Overview The Home Health Registered Nurse is responsible for providing skilled nursing care to patients in their homes while ensuring compliance with clinical standards and organizational policies. This role focuses on admissions, OASIS visits, clinical supervision, and collaboration with the interdisciplinary team to deliver high quality patient-centered care across the assigned coverage area.

Pay Rate:
$ 51-$53 per hour Key Responsibilities Complete comprehensive patient admissions and OASIS assessments in accordance with regulatory and agency guidelines Perform routine and complex skilled nursing visits, documenting care accurately and timely in the electronic medical record Supervise Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs), including clinical oversight and competency support Participate actively in Team Case Conferences, contributing to care planning and interdisciplinary collaboration Assess patient conditions and develop individualized plans of care based on clinical findings Coordinate care with physicians, therapists, caregivers, and internal staff to ensure continuity of services Educate patients and families on disease management, medications, and treatment plans Ensure compliance with all home health regulations, policies, and quality standards Required Qualifications Active Registered Nurse (RN) license Prior home health experience Hands-on experience with Homecare Homebase (HCHB) Current CPR certification Preferred Qualifications Strong wound care experience IV therapy competency Advanced knowledge of OASIS documentation Excellent time management and organizational skills Ability to work independently while managing multiple visits efficiently Pride-Health offers eligible employee's comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance and employee discounts with preferred vendors.
