ELIGIBLE POSITION
•Registered Nurse Job, 12 hour days shift, Admission Discharge Team •Possible weekend option available. Make a weekend premium! Community Healthcare System is hiring a Registered Nurse for the Admission/Discharge Team to work 12 hour days in Munster, IN. The admission/discharge team coordinates care between members of the healthcare team. The admission nurse facilitates the admission process. The discharge nurse facilitates the discharge by coordinating discharge instructions between the physicians, providing for patient education, and assuring medication reconciliation. The admission/discharge nurse plans, implements and evaluates care based on physician orders, current established Standards of Care, the Nurse Practice Act and The Community Hospital policies and procedures.
Registered Nurse Roles and Responsibilities:
Responsible for implementing, directing, communicating and documenting the nursing process. Provides a safe environment for patients, visitors and associates. Practices in accordance with current established standards of care, the Nurse Practice Act, the Powers Health policies and procedures, and supports the mission and philosophy of PH by providing the highest quality care. Provides for the well-being of the community, services the needs of all people and demonstrates the values: dignity, compassionate care, community, quality and stewardship
Required Skills & Qualifications:
Graduate of an accredited school of nursing. Minimum of two years recent clinical experience. Current professional licensure by the State of Indiana Health Professions Bureau. BSN preferred. Obtains and maintains current certification as required in specialty areas.
