Summary Description:
The Registered Nurse will work as part of a multidisciplinary team in the Assertive Community Treatment (ACT) Program serving Adults. The Registered Nurse is responsible for conducting psychiatric assessments; assessing physical health needs; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist and providing a range of treatment, rehabilitation, and support services. The Registered Nurse will support recovery by providing services that reflect our deeply held values that include the recognition of individual rights of self-determination, choice, shared decision-making and collaboration. When possible, the Registered Nurse will utilize evidence-based practices; always embracing a trauma-informed and person-centered approach.Key Responsibilities:
Primary responsibility for the development of initial individualized health assessments on all program participants and ongoing follow-up assessments (every 6 months thereafter). Utilize said assessments to develop, implement, and evaluate the health regimen for 68 recipients.During the assessment, obtains information: a) to establish the degree of medical risk, secure primary medical care and interventions required to promote positive health outcomes, and b) to lead the team in primary care issues.
Participate in determining resources and policies essential to the delivery of nursing care services.
Assess medical care needs and other services to support each participant's wellness.
Collaborate with participants' chart captains to recommend health objectives for recipients service plans.
Facilitate consumer education on prevention, health, wellness, diet, HIV/AIDS education, smoking cessation, and recovery.
Provide education to ACT staff on physical health issues and medical care protocols.
Act as advocate and liaison for recipients in securing medical care, care coordination and other community supports that promote integrated physical and mental health wellness.
Review and audit medication management procedures and teach all ACT staff how to follow and adhere to the procedures.
Consult with community agencies and families to maintain coordination in the treatment process.
Provide after-hours emergency coverage on a rotating basis with clinical staff via a paging system. Document client progress to maintain a permanent record of client activity according to established methods and procedures.
Participate in daily morning team meetings and treatment planning review meetings.
Take the lead role or participate in providing medication administration and medical services.
Under the direction of the team psychiatrist, develop, revise, maintain, and supervise team psychopharmacologic and medical treatment and medication policies and procedures, including transcribing, administering, evaluating, and recording psychotropic medications prescribed by the psychiatrist; evaluate and chart psychotropic medication effectiveness, complications, and side effects; and arrange for required lab work according to protocol.
Under the direction of the team psychiatrist, organize and manage the system of getting medication to clients and integrating medication administration tightly into clients individual treatment plans.
Under the direction of the team psychiatrist, manage pharmaceuticals and medical supplies. Follow the established clinic policies, agency policies, and maintain appropriate professional standards.
Provide 80 percent of treatment contacts in the community.
Use individual client data to inform clinical decision-making and identify areas for quality improvement; use data to track client progress over time. Share client data with clients to help facilitate shared decision-making and keep the client informed of his/her progress Participate in implementing and maintaining overall nursing standards. Competencies:
The RN must have and maintain current CPR/BLS certification.Must possess strong organizational and writing skills. Must also be flexible, respectful, creative, and person-centered. The RN requires a thorough understanding of the process and possibility of robust recovery for people diagnosed with mental health conditions.
Must exhibit patience, flexibility, compassion, and a deep belief in recovery.
Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
Experience understanding family systems based on diversity of cultural and ethnic backgrounds.
Experience understanding various evidence-based models and theories pertaining to psycho-education, motivational interviewing, serious mental illness and recovery.
Extensive knowledge of Assertive Community Treatment, family community support services such as NAMI, and community resources.
Use of Motivational interviewing, recovery and trauma-informed approaches when delivering services.
Highly organized and self-motivated.
Basic computer knowledge and the ability to document in an electronic health record is required. Must be reliable and able to work independently as well as an integral team member, and understand the importance of maintaining confidentiality. Key Performance Indicators (KPIs):
Adherence to treatment protocols and guidelines.
Assessing patient understanding and application of health education provided.
Ensuring all client records are accurate and updated in a timely manner.
Measuring patient satisfaction with nursing care and education received.
Requirements:
Must have a valid Drivers License and a driving record that is satisfactory to our insurance carrier. RN will be using agency vehicles and is responsible for adhering to Agency Driver Policy.Must be a NYS Licensed Registered Nurse.
Must maintain an active license in good standing to practice in New York State.
Must comply with the standards and requirements of all applicable federal, state, local and other laws, rules, and regulations governing medical services.
Must comply with all professional standards and all applicable agency policies and procedures, including the agencys compliance program and code of conduct.
Understanding of 42CFR, HIPAA Privacy Standards, and related agency policies.
Must maintain pre-employment clearances by applicable regulatory clearances as required.

Special Considerations:
The position may require extended periods of sitting and computer-based work.Work hours, reporting structure, and on-site location are subject to change based on program needs.
