Job Description

Category RN- Case Management Job Type Contract Make a meaningful impact as an RN Case Manager, contributing to a patient-centered care environment through assessment, facilitation, and advocacy for individuals and families with complex health needs. This contract position offers an opportunity to manage care across the continuum, ensuring high-quality, resource-effective outcomes for patients requiring acute inpatient case management.
Schedule:
Five 8-hour day shifts per week Every other weekend required
Preferred Qualifications:
Unrestricted RN license Minimum of 2 years' recent, acute inpatient case management experience Strong clinical assessment, communication, and critical thinking skills Ability to collaborate as part of an interdependent, multidisciplinary team Experience with motivational interviewing, self-management support, and patient-centered care planning Familiarity with care transitions and population health concepts
Responsibilities:
Complete comprehensive assessments of patients and their support systems, evaluating biomedical, psychological, and social needs Develop and implement personalized care plans to enhance patient experience and outcomes Engage patients and families through education, advocacy, and ongoing communication Employ professional judgment and critical thinking to address barriers and promote quality care Guide patients and families through health care decisions, including advanced care planning and end-of-life considerations Coordinate seamless transitions between care settings and advocate for optimal use of resources within patients' health plans Collaborate with interdisciplinary team members to manage complex cases and facilitate care conferences Monitor and adjust care plans in response to changing patient needs Participate in process improvement initiatives and interventions supporting patient safety and organizational goals Facilitate and address cases involving abuse, neglect, and other sensitive situations This is a rewarding opportunity for experienced RN Case Managers committed to patient advocacy, multidisciplinary collaboration, and enhancing health outcomes.
