Job Description

Job Summary We are seeking a dedicated and compassionate Registered Nurse specializing in Long Term Care (LTC) to join our dynamic healthcare team. In this vital role, you will provide expert nursing care to residents in a long-term care setting, ensuring their comfort, safety, and well-being. Your expertise will support residents with complex health needs, including memory care, hospice and palliative medicine, and chronic disease management. This position offers an exciting opportunity to make a meaningful difference in the lives of seniors and individuals requiring ongoing medical support. You will work collaboratively with interdisciplinary teams to deliver personalized care that promotes independence and enhances quality of life. Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and functional evaluations to determine individualized care plans. Administer medications, injections, IV infusions, and perform catheter care following aseptic techniques and HIPAA regulations. Monitor patients' conditions closely, managing acute pain, respiratory support (ventilators), and infection control procedures. Coordinate discharge planning, case management, and collaborate with physicians on diagnostic evaluations such as laboratory testing and imaging (e.g., sonography). Provide specialized care for residents with memory care needs, behavioral health issues, developmental disabilities, or those requiring hospice or palliative services. Maintain accurate medical documentation within electronic health record (EHR) systems such as Epic or Cerner; ensure compliance with NCQA standards and ICD coding protocols. Support patient education initiatives including health coaching on chronic disease management, nutrition, medication administration, and mobility assistance. Requirements Valid RN license in good standing; current CPR/BLS certification required. Proven experience in long-term care settings such as nursing homes or assisted living facilities. Critical care experience preferred, including ICU or emergency room exposure; familiarity with Level I Trauma Center protocols is advantageous. Demonstrated proficiency with EMR/EHR systems like Epic or Cerner; knowledge of
ICD-10/ICD-9
coding is a plus. Strong clinical skills in IV insertion, suturing, ostomy care, dialysis support, tube feeding management, and aseptic techniques. Ability to work effectively with diverse populations including seniors, individuals with disabilities or developmental challenges, and those requiring complex medical interventions. Excellent communication skills for triage assessments and interdisciplinary collaboration; physical ability to perform patient transfers using Hoyer lifts or similar equipment.
