EDUCATION AND EXPERIENCE
: Registered Nurse with current license; and Administrative experience in human services or related field including experience in quality assurance/improvement and site development; and Infant mental health experience or education recommended but not required; Grant research and writing recommended but not requiredALSO REQUIRED

Registered Nurse Manager:
Manage Docusign enrollment process Maintain online client file consent and enrollment system for department integration Assist the Director of Finance and Human Resources in the hiring and training of all family services program staff in the department. Provide grant required supervision of Team Leads Represent the department and establish effective working relationships with a variety of community governmental and professional organizations to facilitate coordination/collaboration, and achievement of program/agency goals and objectives. Serve as HIPPA Liaison by reviewing all incoming and outgoing releases of information for accuracy and content requested Provide input into the annual evaluation and supervision of department staff Solicit regular customer feedback for continuous quality improvement Maintain a high degree of confidentiality of patient information Exhibit a commitment to exemplary customer service Assist with emergency and epidemic surveillance activities as necessaryRN DUTIES SPECIFIC TO EACH FUNDING STREAM
FCI- Family Connects Illinois
- Provide professional one-on-one clinical consultation and technical assistance to Family Connects RN's following program guidelines and funder requirements using a reflective supervision model Create weekly case conference and team meeting agendas, host meeting and prepare minutes Assist in conducting FCI home visits as needed Conduct monthly Quality Assurance procedures including client chart review, analysis of scheduling and home visit completion rates, rounding statistics and progress toward funder performance measures Conduct quarterly nurse fidelity checks by shadowing a home visit• Attend bi-monthly nursing supervisor calls Update Policy and Procedure Manual• Complete Fidelity Assessment Tool Maintain Medical Maternal and Infant Health Guidelines and update bi-annually Develop and maintain relationship with Medical Director Update annual health system linkage agreements Complete Quarterly Site Support Plans and ensure alignment with rest of department to maintain service integration and common goals toward reaching the mission Order client incentives in accordance with budget availability Provide quarterly report to Community Alignment Board (AOK Network) BBO-C•Better Birth Outcomes•Comprehensive Provide professional one-on-one clinical consultation and technical assistance to RN's and BBO Navigator staff Coordinate initial and annual education requirements for BBO staff per Bureau of Maternal & Child Health requirements (including Mandated Reporter and Unconscious Bias) Create, implement and update community outreach plan Maintain inventory tracking tool Serve as liaison to MCH State staff• Conduct Quality Assurance of daily clinical operations managing client caseload Develop and maintain internal Policy and Procedure Manual Maintain BBO Resource Guide Complete and submit quarterly Periodic Performance Review (PPR) report
Director of Family Services:
All duties assigned as a Registered Nurse Manager in addition to the following: Coordinate all family services programs and operations to ensure integrated, comprehensive service experience for all enrolled families; maximizing efficiency, effectiveness, and excellent customer service Manage internal coordination of services through use of the online IRIS referral system Work with Director of Finance and Human Resources to develop budgets, approve/monitor expenditures, ensure revenues are properly accounted for, and consistent with projections for all family service programs. Develop and update department-wide marketing materials with funder required branding Establish and maintain departmental policies and procedures in accordance with grant requirements- Support development and execution of growth strategy and scaling plan for all Family Services Programs Develop and maintain communications with funders, seek grant opportunities and manage large-scale donation and funding streams for the Health Department as a whole.
DUTIES SPECIFIC TO EACH FUNDING STREAM
Doula- Provide support to Doulas outside of typical office hours and serve as back-up doula in the event no doula is available to attend a birth
- Provide one-on-one supervision, consultation, and technical assistance to Doulas using reflective practice
- Create and implement client incentive program that aligns with budget availability and FANA required activities
- Ensure site support plan aligns with rest of department to ensure service integration and common goals toward reaching the mission MIECHV
- Maternal Infant Early Childhood Home Visiting (Coordinated Intake) Build and maintain relationships with community health system providers to enroll families in services as early in their pregnancy as possible Ensure site support plan aligns with rest of department to ensure service integration and common goals toward reaching the mission Create, implement and update community outreach plan MBD
- Moving Beyond Depression Maintain contract with Supervising organization
- Complete and submit quarterly reports to funder
- Order incentives that align with budget availability AOK

Pay:
$70,000.00- $74,000.
