Job Description

Company DescriptionDo you have HEART? We are looking for individuals who can embrace our mission to purposely brighten and enrich the lives of those we serve with HEART; Hospitality, Excellence, Appreciation, Respect & Teamwork. This individual is responsible for the daily clinical aspect functioning to meet all residents' needs.

Job DescriptionResponsible for medication supervision/administration in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.

May be responsible for the admissions of new residents to include all admission assessments and documentation.

Responsible for completing acute and long-term care plans or service plans. May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the Director of Nursing.

May be responsible for discharges, assessments, and clinical documentation as indicated. Provide general consultation regarding health-related concerns within the community.

Notify physicians and/or family members of any change in resident's health and provides proper documentation.

Respond quickly to all emergencies.

Maintain close communication with all departments and staff members.

Maintain confidentiality of residents' and prospective residents' information.

Understand role in the Safety & Disaster Plan.

Provide educational meetings for residents and staff.
QualificationsLevel of Formal Education:
An Associate's Degree or equivalent from a two year college or technical school: or Two years related experience and/or training equivalent combination of education and applicable experience.

Type of Experience:
skilled, sub-acute, or memory care setting
Special Certifications:
Must have a Registered Nurse's License current, with no judgments against it, in the state in which he/she is practicing and must be CPR certified
Language Skills:
Acceptable fluency in English in order to perform job duties and speak, read, write, and communicate with supervisors, coworkers, residents, and applicable third parties.
Technical Competencies:
Basic computer skills
Skills and Ability:
 Ability to work as part of a team and handle multiple tasks safely and effectively.
