Registered Nurse (RN)

1. Interview patients, documents their medical history, and conducts assessments of vital signs, (e.g. blood pressure, temperature, weight/BMI) as required by clinical protocols and grantee data collection standards. Utilizes evidence-based practices and ensure a Patient Centered Medical Home approach. 2. Maintain proper clinical records and reports using problem-oriented medical records. 3. Assist with providing patient care, discuss and record chief health concerns, perform tests, and administer injections via established program protocols. 4. Provide specific health counseling and general health education to individual patients and/or groups. 5. Assist patients with scheduling appointments for specialized medical care beyond the scope of the clinic services. 6. Cooperate with other health care providers (e.g., behavioral health staff) to ensure a holistic treatment plan. 7. Understanding of Substance Use Disorders. 8. Participate in staff meetings and Integrated Treatment Team meetings to review patient care. 9. All other duties as assigned.

MINIMUM QUALIFICATIONS

Must possess a Nursing Degree or diploma from an accredited college or university, must have a current, valid RN license in the state of Kentucky and current CPR certification. Must have a valid Kentucky driver's license, must pass background check.

SPECIAL KNOWLEDGE, SKILLS AND ABILITIES

Must be able to make independent decisions when warranted; must be knowledgeable of nursing and medical practices and procedures as well as laws guidelines, regulations that pertain to quality patient care; must have strong clinical skills, and must have experience working with and sensitivity to the homeless/low income population and persons with behavioral or substance use health care disorders and knowledge of the Appalachian culture.
