$150 - $200 per visit + mileage reimbursement! Expand your expertise in transitional care, home health nursing, and OASIS documentationPrimary Responsibilities:
Perform comprehensive Start of Care nursing assessments Complete high-quality OASIS documentation with accuracy and timeliness Develop individualized care plans based on patient goals, safety needs, and medical conditions Educate patients and their families on expectations, medication management, and resources Partner with therapists, case managers, and providers to ensure seamless care coordination Serve as a clinical leader who shapes the patient's home health experience from day oneRequired Qualifications:
