Job Description

Position Summary The Resident Care Coordinator (RCC) plays a vital role in supporting the day-to-day clinical and operational functions of the community. This position serves as a liaison between residents, families, providers, pharmacy, caregivers, medication technicians, and leadership to help ensure quality resident care, regulatory compliance, accurate documentation, and efficient staffing operations. The RCC assists with resident admissions, medication and chart audits, provider communications, scheduling coordination, employee onboarding, and ongoing follow-up tasks to support smooth community operations. This position requires strong organizational skills, attention to detail, multitasking ability, professionalism, and a proactive approach to problem-solving. The RCC works closely with the Director of Nursing (DON) and Executive Director (ED) and may assist in additional operational or clinical support duties as assigned. Essential Duties and Responsibilities Clinical Coordination & Resident Care Assist with coordination of resident care needs and services. Coordinate care and communication with physicians, pharmacies, hospice, home health, podiatry, behavioral health providers, and outside medical providers. Process physician orders and ensure timely follow-up on faxed communications and outstanding orders. Assist with care plan and ISP updates by gathering required documentation and resident information. Coordinate resident appointments and transportation arrangements as needed. Assist with resident transitions to and from hospitals, emergency rooms, or outside appointments. Monitor and follow up on pharmacy communications and medication-related concerns. Admissions & Move-In Coordination Assist with all new resident admission processes. Ensure resident charts are properly created and maintained. Follow admission checklists to ensure all required documentation is completed and received. Coordinate pharmacy setup and ensure all admission documentation is transmitted and acknowledged. Verify medication orders and MAR setup are completed prior to resident move-in whenever possible. Update shower schedules, laundry schedules, and resident service tracking systems as needed. Coordinate ordering of resident-specific supplies for private pay and Medicaid residents. Medication & Documentation Oversight Conduct routine MAR audits and report findings to nursing leadership. Perform chart note audits to ensure required documentation is completed accurately and timely. Monitor for appropriate charting related to alert charting, ER transfers and returns, pharmacy communications, family communications, refusals of care, and significant resident concerns or changes. Follow up with medication technicians and staff regarding missing or incomplete documentation. Report repeated documentation concerns to nursing leadership for corrective action when necessary. Ensure medication carts remain clean, organized, and compliant. Audit narcotic books and medication storage areas for accuracy and compliance. Ensure discontinued or unused medications are appropriately destroyed or returned to pharmacy according to policy and regulations. Staffing & Scheduling Support Prepare monthly staff schedules for assigned buildings by the 20th of the preceding month. Monitor schedules proactively to identify staffing gaps and hiring needs. Coordinate and track employee time-off requests and schedule coverage needs. Monitor shift trades and staffing adjustments to minimize unnecessary overtime. Ensure overtime is approved by the Executive Director prior to scheduling whenever possible. Maintain and update employee call-out logs and attendance tracking. Assist with recruiting, interviewing, and scheduling candidate interviews. Assist with onboarding and orientation processes for newly hired staff. Compliance & Administrative Duties Track employee credentialing, training, and state-required certifications. Maintain spreadsheets and tracking systems related to employee compliance requirements. Monitor follow-up items and ensure timely completion of outstanding tasks. Assist with maintaining survey readiness and compliance standards. Address routine resident and family concerns professionally and promptly. Maintain communication with leadership regarding operational, staffing, or resident care concerns. Routine Responsibilities Daily Responsibilities Review and respond to after-hours communications and emails as needed. Monitor staffing schedules for call-outs, coverage needs, or scheduling concerns. Process physician orders and ensure documentation is forwarded appropriately. Review charting completion and follow up on missing documentation. Coordinate resident transportation and appointments as needed. Update attendance and call-out tracking logs. Weekly Responsibilities Complete medication cart audits and medication storage inspections. Order resident supplies for private pay and Medicaid residents. Follow up on outstanding physician orders and fax communications. Monitor medication destruction and pharmacy return processes for compliance. Scheduled Audit Responsibilities Complete MAR audits on designated audit days and communicate findings to nursing leadership. Conduct ongoing documentation and compliance reviews to support quality assurance efforts. Qualifications Previous experience in assisted living, memory care, long-term care, or healthcare preferred. Experience with medication administration systems, documentation systems, and resident care coordination preferred. Strong organizational skills and ability to multitask in a fast-paced environment. Ability to maintain professionalism and confidentiality. Strong communication and follow-up skills. Ability to work collaboratively with residents, families, providers, and team members. Knowledge of assisted living regulations and healthcare documentation practices preferred. Physical Requirements Ability to move throughout the community as needed. Ability to sit, stand, bend, lift, and assist with routine operational duties. Ability to respond appropriately during emergencies. Additional Information This position may require flexibility with scheduling, occasional on-call support, and assistance with operational needs based on community census and staffing needs. Duties and responsibilities may be adjusted at the discretion of leadership to meet the needs of the community.
Pay:
$24.00 - $26.00 per hour
