We're looking for a Registered Nurse (RN) to step into a Resident Care Manager (RCM) role that blends clinical leadership, admissions involvement, and hands-on support. This isn't your typical desk-only position—you'll be a key part of both the care team and the leadership team. What You'll Do As an RCM, you'll guide care for your assigned residents while supporting daily operations and helping ensure smooth new admissions. Care Coordination Oversee resident care from admission through discharge Complete and review assessments (including MDS) and develop meaningful care plans Monitor changes in condition and ensure timely follow-up Partner with physicians, therapy, families, and interdisciplinary teams Lead and participate in care conferences Admissions & Transitions Assist with admissions, ensuring smooth, safe, and welcoming transitions Review referrals and collaborate with the admissions team Help coordinate initial assessments, orders, and care planning Support occasional admissions after hours or on weekends Team Support & Leadership Serve as a go-to clinical resource for nursing staff Guide, mentor, and support your team day-to-day Promote best practices and high-quality care delivery Jump in where needed to keep things running smoothly Compliance & Quality Maintain accurate, timely, and survey-ready documentation Ensure compliance with state, federal, and facility standards Participate in QAPI and quality improvement efforts Daily Operations Stay connected to what's happening on the floor and support operations as needed Help ensure consistent, high-quality resident care Provide support for weekend coverage on a rotating or as-needed basis Participate in an on-call rotation Assist with direct resident care when needed to support safe and effective operations What You Bring Active Washington State RN license (required) Experience in skilled nursing or long-term care preferred MDS and care planning experience a plus Strong organizational and communication skills A team-first mindset and willingness to pitch in Physical Requirements Ability to stay active throughout the day (standing, walking, bending) Comfortable assisting residents with mobility and transfers Ability to lift or support up to 50 lbs, with or without assistance What Makes This Role Stand Out A mix of leadership, admissions involvement, and hands-on care A supportive, team-oriented environment Meaningful impact on residents' lives every day Variety in your workday—no two days are exactly the same Schedule & Expectations Full-time position Participation in weekend coverage rotation Involvement in admissions, including occasional off-hours needs Participation in an on-call rotation Flexibility to meet the needs of residents and the care team Join Our Team If you're an RN looking for a role where you can lead, stay connected to resident care, and play a key role in both daily operations and admissions, this is it.
