Job Description

Category RN- Case Management Job Type Contract Make a meaningful impact as part of a multidisciplinary care team as you deliver comprehensive case management services within an acute inpatient setting. This contract position demands strong clinical expertise and a compassionate, patient-centered approach to drive optimal health outcomes across diverse patient populations.
Schedule:
Five 8-hour day shifts per week Every other weekend coverage required
Key Qualifications:
Active RN license required Minimum of 2 years' recent acute inpatient case management experience (mandatory) Strong clinical assessment and care coordination skills Expertise in motivational interviewing and supporting patients through complex care transitions Ability to collaborate with multidisciplinary teams and engage patients and families as partners in care Proficiency in care planning, resource identification, and advocacy Outstanding interpersonal, written, and verbal communication skills
Primary Responsibilities:
Perform comprehensive assessments of patient and family needs (biomedical, psychological, social, and functional) Create and implement individualized, patient-centered care plans focused on safety, quality, and cost-effectiveness Engage patients and families through ongoing communication, education, and advocacy Address complex management needs for critical events or diagnoses, ensuring coordination and safe care transitions Provide counseling on treatment options and advanced care planning, including end-of-life decision support Collaborate with physicians, specialists, and social workers to facilitate seamless care Support process improvement initiatives to reduce risk and enhance patient outcomes Intervene and facilitate support in cases involving abuse and sensitive family dynamics
