POSITION DUTIES

•Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization

MINIMUM REQUIRED QUALIFICATIONS

•FL/compact RN license; BLS; 2 years case manager experience in an acute hospital setting; Acute hospital experience in specialty during the past year with no more than a 90-day break; Knowledgeable in Meditech, Midas, and Interqual.

LENGTH OF ASSIGNMENT

•13 weeks

SHIFT / HOURS PER WEEK

•8:00AM-4:30PM with every other weekend SYSTEMS•Meditech, Midas, Interqual
