RN Case Manager Home Health

We're seeking a high energy, organized, and caring RN Case Manager to join our Roseville team! The ideal candidate has the following traits: Registered Professional Nurse, a graduate of an approved school of professional nursing who is currently licensed as a registered nurse in California Minimum 2 years acute care experience or equivalent education commensurate (Required) Previous Home Health care case management experience preferred Care and passion for providing exceptional patient care Highly energetic and organized individual who has a positive, "take ownership" attitude Critical thinking skills and clinical knowledge to create a viable and effective patient transition plan and identify delays in service, or quality issues that impede the progression of care. Strong clinical assessment skills necessary to provide utilization review and transition planning services to meet the patients' complex medical, emotional and social needs
