Day Work Week:

Must possess a current, unencumbered nursing license in the state of practice. Job Responsibilities (Includes but not limited to the following)
Patient Care:
Provide skilled nursing services in accordance with physician orders and the plan of care in the patient's home. Responsibilities include the observation, assessment, nursing diagnosis, counsel, care, and health teaching of the hospice patient and family.
Assessment:
Perform and document initial and ongoing assessments of the patient's physical, functional, psychosocial, and environmental needs, as well as the impact on activities of daily living.
Plan of Care:
Initiate the IDT (Interdisciplinary Team) Plan of Care in consultation with the IDT members, attending physician, and medical director. Review and revise the plan as necessary. Coordinate with SNFs for facility-bound patients.
Service Coordination:
Determine the scope and frequency of services needed based on patient acuity and family needs.
Eligibility:
Apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient's eligibility.
Admissions:
Participate in the shared RN responsibility for admitting patients, as the organization admits patients 24/7, 365 days a year.
Caregiver Support:
Assess the caregiver's ability to meet the patient's immediate needs upon admission and throughout care. Evaluate and document the appropriateness and ability of the patient or family to administer medications.
Evaluation:
Evaluate the effects of care provided and regularly re-evaluate the patient's nursing needs.
Skilled Nursing:
Provide services requiring substantial specialized nursing skill based on physician orders and the nursing diagnosis related to palliative and end-of-life care.
Examples include:
Physical assessment Managing discomfort and providing symptom relief Patient and family teaching Urinary catheter insertion and care Venipuncture and administration of IV fluids or medications Sterile and non-sterile dressing changes Appropriate preventive and rehabilitative nursing procedures
Education:
Educating the patient and family regarding the disease process, including self-care techniques, end-of-life care, and processes for dealing with issues of ethical concern. Equal opportunity employer
