The Clinical Case Management RN Case Manager assesses the physical, functional, social, psychological, environment and financial needs of clients/patients; identify a cost -effective comprehensive plan to meet the families' service needs, and implement the plan.SUPERVISION
Reports directly to Director of Clinical Case ManagementRESPONSIBILITIES AND DUTIES
Provide referrals to appropriate community resources. Facilitate access and communication when multiple services are involved. Monitor activities to ensure that services are actually delivered and meet the needs of clients. Coordinate services to avoid duplication. Assess the clients' formal and informal support systems. Monitor clients progress toward goal achievement and periodically reassess changes in health status. Monitor the care plan to ensure the effectiveness and appropriateness of services. Act as patient advocate; identify and develop new community resources; assist with problem solving. Communicate clinical information to payers to authorize days for billing. The incumbents may be requested to perform job-related tasks other than those stated in this description.EDUCATION AND WORK EXPERIENCE
Graduate of an approved School of Nursing, BSN preferred, Case Management certification preferred. Minimum of three years med/surg experience in an acute care hospital. Computer skills at data entry level. Current Louisiana RN License or Compact State License and CPR certified. Knowledge of community resources and programs. Physical Demands/Work Environment Work requires a variety of physical activities, including moving about within and outside of all hospital properties for long periods of time. Must be able to respond quickly and effectively to emergency and non-emergent situations. May be required to assist in controlling disorderly conduct or combative patients. Must be able to exchange accurate information with patient, family, peers and medical personnel.
