RN for LTC Assessments/Visits for Medicaid, Per Diem $75 - $150 per assessment/visit

We are looking for RNs and LPNs that have experience performing initial assessments, routine visits and reassessments for Medicaid insured clients needing Long Term Care .

• This is not an hourly position
• . This person is paid $150.00 per INITIAL Assessment and $125.00 for the Re-Assessments. $75-$100 for routine visits We offer Mileage Reimbursement as well There are NO set amount of hours per week that you work, Nor are there a certain amount of assessments that are scheduled every week.
• This position is on an as needed basis.
• Individual must have access to a printer as well as a scanner, and have reliable transportation.

Responsibilities:

Supervising and evaluating the performance of Personal Care Aides Evaluating clients Level of Care and initiating and modifying the Plan of Care as needed Performing assessments to initiate services for new clients as well as perform re-assessments Completing DMAS forms 97A/B, DMAS 98R, DMAS 99
