  • Full-time Visiting Nurse Home & Hospice is seeking an experienced RN for their Intake Team.
The Intake RN is responsible for coordinating the intake and referral process for home healthcare and hospice services, including reviewing clinical and insurance information, coordinating admissions, verifying benefits and authorizations, and maintaining accurate EHR (electronic healthcare record) documentation. The role serves as a liaison between referral sources, hospitals, patients, and internal teams to ensure timely, compliant, and high-quality patient care coordination. Supports customer service, intake workflow management, and regulatory compliance while assisting with referral follow-up, hospital holds, and process improvement initiatives. The role is a full-time, 40-hour per week, position with 8AM-4:30PM hours or similar at our Middletown office. There is a rotating weekend commitment of every 3rd weekend. What will you do as an Intake RN? Manage home healthcare referrals and intake coordination Review clinical, insurance, and demographic information for admissions Coordinate timely patient admissions with clinical and scheduling teams Verify insurance benefits, authorizations, and identify coverage issues Serve as liaison between hospitals, referral sources, patients, and staff Maintain accurate EHR documentation, referral tracking, and communication notes Monitor hospitalized patient referrals and manage hospital hold processes Ensure compliance with HIPAA, payer guidelines, and agency policies Provide excellent customer service and support intake process improvements Assist with administrative duties and intake workflow management as needed What are the qualifications to be an Intake Registered Nurse (RN)? Excellent interpersonal and telephone communication skills. Knowledge of agency programs, policies, and procedures. Ability to work cooperatively with clinical administrators, managers, and staff within the agency. Strong critical thinking and problem-solving skills. Ability to organize workload in a busy environment and manage competing priorities. Attention to detail and accuracy in capturing and documenting referral information. Ability to work with highly confidential information. What Professional Standards are expected?

What kind of Education/Experience do I need? Graduate of an NLN approved program in Nursing. Maintain RN licensure in Rhode Island and Massachusetts. If a Massachusetts RN license is not held at time of hire, the employee will have the first year of employment to obtain it
