Centralized HR, IT, education, and employee experience departments for improved support. What You'll Do The PSC facilitates delivery of best-in-class customer experience to all customers. Andwell's customers include patients and their families, Referral Partners, employees, donors and Board Members, health insurance companies, and the communities we serve. Provide leadership and serve as a role model in office procedures and customer service standards. Create office systems and initiate office procedures that are efficient, cost effective, and consistent with Organization standards. Receive and document referral information, medical orders. Assign diagnosis codes, and process referrals in a timely manner to appropriate service line for admission. Work cooperatively with social workers, discharge planners, nurses, and physicians in assessing and planning for home care Collect and document information about the patient's situation and functional capacity, relative to the care plan post discharge. Collaborate with appropriate Clinical Supervisors, referral sources, provider offices, community partners, and the patient, to identify and meet any care needs/concerns prior to admission. Demonstrate an understanding of third-party reimbursement criteria for home health, hospice, palliative care and supporting services, including, but not limited to, Medicare, Medicare Advantage, MaineCare, HMOs, and private insurances. Possesses knowledge of health care delivery systems, including Organization service lines and ancillary programs. Ability to interpret these services to patients and to other community partners. Maintain patient records in accordance with established protocols and policies Complies with Conditions of Participation, informed consent, rights to participate in care decisions, confidentiality, and advanced directives. Understands and provides support for all regulatory initiatives (i.e.: F2F Encounters) Follows policies/procedures regarding Patient Rights and Responsibilities, Ethics, Care, Treatment, Service and Management of Information. Initiate calls to referred patients, to ensure timely coordination of care and delivery of services. Coordinate communication and documentation related to patient transfers to in-patient facilities. What You'll Bring You will effectively leverage all available resources to help our customers understand and navigate the home healthcare referral process by providing assistance to customers telephonically, electronically, or by other organization-approved means; collecting, entering, and updating patient information in our Electronic Medical Record database. RN active license in State of Maine 2 years recent nursing experience 1-2 years nursing experience in the community health setting preferred; Basic computer knowledge, data entry experience required Working knowledge of medical terminology and healthcare insurance authorization processing preferred.
