Job Summary We are seeking compassionate and dedicated Registered Nurse (RN) to join our team on a per diem basis for home health visits. M
In this vital role, you will provide personalized nursing care directly in patients' homes, ensuring they receive the highest quality of medical attention and support. Your expertise will help improve patient outcomes, promote independence, and enhance quality of life for individuals across diverse care settings. This flexible position offers the opportunity to make a meaningful difference while working independently in a dynamic environment. Responsibilities Conduct comprehensive patient assessments and monitor vital signs during home visits to ensure optimal health status. Administer medications, manage catheter care, and assist with activities of daily living (ADLs) such as bathing, dressing, and mobility support. Document medical records accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, maintaining compliance with HIPAA regulations. Assist patients with dementia, Alzheimer's disease, or memory care needs by implementing specialized care plans tailored to their condition. Provide hospice and palliative care support, ensuring comfort and dignity for terminally ill patients. Utilize medical terminology effectively to communicate patient conditions and treatment plans with healthcare teams. Operate medical equipment such as Hoyer lifts and perform wound care procedures when necessary. Skills Proven experience in home health nursing with a focus on senior and disability care. Strong patient observation skills to detect changes in health status promptly. Familiarity with EMR/EHR systems for accurate documentation of patient information and medical records. Knowledge of assisting with ADLs, medication administration, catheter care, and hospice or dementia-specific care techniques. Ability to work independently while maintaining excellent communication with healthcare teams and families. CPR certification and understanding of medical terminology are required; additional certifications such as Alzheimer's or memory care training are a plus. Experience working with individuals with developmental disabilities or disabilities is highly valued.
