ACO REACH
program. This role coordinates onboarding, communication, and day-to-day provider support while managing administrative and organizational tasks that ensure providers and internal teams remain aligned and on track. Key Responsibilities Support provider onboarding, engagement, and ongoing communication within theACO REACH
program Assist the Director of Provider Relations with daily provider inquiries and project coordination Track tasks, deadlines, and provider information to ensure timely and accurate follow-up Send routine reminders and communications to practices on a daily, weekly, monthly, quarterly, and annual basis Create and distribute monthly provider newsletters Respond to provider questions and escalate complex or higher-level issues to the Director prior to resolution Attend meetings on behalf of the Director when requested Collaborate with internal teams to ensure consistent and accurate provider communication Travel up to 50% with the Director of Provider Relations as needed to support provider engagement and onboarding Decision-Making & Reporting Maintains clear ownership of assigned responsibilities Works collaboratively with the Director and reports complex issues before responding Qualifications College degree preferred but not required Strong organizational, time-management, and attention-to-detail skills Excellent written and verbal communication abilities Ability to manage multiple priorities in a fast-paced environment Experience in healthcare administration, provider relations, or a related field preferredJob Type:

Have you supported provider onboarding, credentialing, practice engagement, or provider communications? How comfortable are you communicating professionally with physicians, practice managers, and healthcare executives? Are you familiar with value-based care programs, ACOs, Medicare Shared Savings Programs, orACO REACH?
Are you comfortable managing multiple physician practices, ongoing requests, deadlines, and follow-ups at the same time? Do you have prior healthcare experience or knowledge related to primary care, medical practice operations, medical coding, care management, or a similar healthcare setting? Are you willing and able to travel as needed to physician practices to meet with providers, provide training on platforms and workflows, and assist with practice onboarding and support? Do you have experience working directly with healthcare providers (physicians, practices, clinics, hospitals, etc? Are you proficient with Microsoft Office, including Outlook, Word, and Excel?Experience:
