Click on How to Apply OJT- Community Relations Coordinator The Community Relations Coordinator (CRC) is the entry-level field and community relations role at Senior Helpers of Polk County. This role is focused on building and supporting referral source relationships that generate qualified client leads for the Client Services Manager. The CRC begins by supporting existing referral source relationships alongside the Client Services Manager and grows into independently owning an assigned territory. All referral-sourced leads are passed directly to the Client Services Manager for inquiry follow-up and assessment. The CRC reports to the Client Services Manager and works in close alignment with the Director of Operations on activity goals and territory strategy. Primary Responsibilities A. Referral Source Support & Relationship Development
  • Accompany the Client Services Manager on referral source visits during Phase 1 to learn the territory, build relationships, and develop an understanding of each referral source's needs and preferences.
  • Conduct cold calls to prospective new referral sources as directed by the Client Services Manager; introduce Senior Helpers services professionally and document outcomes in the CRM system.
  • Maintain consistent follow-up cadence with assigned referral sources; ensure no relationship goes cold due to lack of contact.
  • Identify new potential referral sources — hospitals, nursing homes, assisted living facilities, memory care communities, rehab centers, physicians' offices, veteran organizations, and others — and communicate findings to the Client Services Manager.
  • Pass all referral-sourced leads to the Client Services Manager promptly and with complete documentation; ensure the Client Services Manager has everything needed to follow up without delay. B. Marketing Activity & Event Coordination
  • the monthly marketing calendar as directed by the Client Services Manager; coordinate logistics for networking events, trade shows, community presentations, and Senior Helpers-sponsored programs.
  • Coordinate and assist with Virtual Dementia Tours and Veteran Pinning Ceremonies; manage scheduling, communication, and on-site logistics.
  • Represent Senior Helpers professionally at community events, networking groups, and industry organizations; look for opportunities to grow the brand's visibility in the Polk County market.
  • Assist in developing and distributing marketing materials to referral sources as directed. C. CRM System & Reporting
  • Maintain accurate and current records for all referral source contacts and activity in the CRM system; document every visit, call, and touchpoint with outcome notes.
  • Track weekly activity metrics — visits completed, cold calls made, new sources identified, leads passed to Client Services Manager — and report to the Client Services Manager weekly.
  • Prepare and submit the monthly referral activity report to the Client Services Manager on time; include referral source visit counts, new sources added, leads generated, and any relationship flags. D. Compliance & Brand Standards
  • Adhere to all AHCA, JCAHO, and Florida state statutes, rules, and company policies in all external interactions.
  • Represent the Senior Helpers brand with professionalism, warmth, and consistency at all times in the field and community.
  • Maintain confidentiality of all client, caregiver, and company information at all times. E. Weekly Activity Tracking & SMART Goals Reporting
  • Complete and submit the Senior Helpers CRC Weekly Activity Tracking Sheet every week by end of day Sunday; this document tracks all field activity against point goals and must be submitted to the Client Services Manager without exception.
  • The weekly activity tracker has a goal of 100 total points; the CRC is expected to understand the point values assigned to each activity type and plan their week accordingly to hit or exceed the target.
  • Participate in the monthly SMART Goals review with the Client Services Manager; come prepared with completed actuals for all tracked categories including referrals received, new client starts from referral sources, marketing connections, lead follow-ups, WellSky data entry, action plan usage, new referral source creation, social media posts, and event attendance.
  • Submit a monthly Sales Plan on time as required by the SMART Goals scorecard.
KPI Framework — Weekly & Monthly Reporting The Community Relations Coordinator's performance is tracked through two tools: the Senior Helpers Weekly Activity Tracking Sheet (submitted weekly) and the Senior Helpers SMART Goals Scorecard (reviewed monthly). The Client Services Manager incorporates CRC data into the bi-weekly report to the Director of Operations. KPI targets below align directly with the corporate scoring framework. Point values and bonus thresholds are defined in the Senior Helpers SMART Goals for the Salesperson scorecard. The CRC is expected to review this document, understand the scoring, and discuss performance against it monthly with the Client Services Manager. During Phase 1, targets will be adjusted to reflect supported activity alongside the Client Services Manager. Career Progression Framework The CRC role is designed as the entry point into a Community Relations Manager career path. Progression from Phase 1 to Phase 2 — and ultimately to the CRM title — is based on demonstrated performance, not tenure. The Client Services Manager and DOO will evaluate readiness together and communicate advancement expectations clearly. Phase Focus Milestone to Advance Phase 1 Foundation Accompany CSM on referral source visits; support event coordination and marketing calendar execution; maintain CRM data entry; conduct cold calls to new referral sources under CSM guidance; coordinate Virtual Dementia Tours and Veteran Pinning Ceremonies logistics. Consistent activity metrics met week over week; strong referral source rapport demonstrated; CRM system maintained accurately; Client Services Manager confidence in CRC's representation of the Senior Helpers brand in the field. Phase 2 Full Ownership Independently manages assigned referral source territory; owns the full referral funnel from first visit through lead generation and Client Services Manager handoff; develops new referral sources proactively; presents monthly referral activity report to CSM and DOO; seeks leadership positions in community organizations to grow brand presence. Sustained independent performance across all referral KPIs; referral-sourced leads consistently feeding Client Services Manager pipeline; Client Services Manager and DOO alignment that full territory ownership is appropriate. JD updated to CRM title upon advancement. Qualifications
  • Minimum two (2) years of experience in community relations, sales, marketing, healthcare outreach, or a related field preferred.
  • Experience in home care, senior services, or healthcare strongly preferred.
  • Demonstrated ability to build and maintain professional relationships with external partners and referral sources.
  • V erbal communication and presentation skills; comfortable representing the brand in community settings.
  • O rganizational skills and follow-up discipline; ability to manage a territory and activity calendar independently.
  • Proficiency in CRM systems; ability to learn new platforms quickly required.
  • Proficiency in Microsoft Office Suite.
  • Valid Florida driver's license if operating company vehicle, access to reliable personal transportation, and willingness to travel throughout the Polk County service area daily.
  • Ability to work independently and as part of a collaborative team under the direction of the Client Services Manager.
