Base salary range

Licensure/Education Need:

LPN or RN Preferred OR Bachelor's degree in healthcare, marketing, or related field (with relevant experience) 3

Most Critical Qualifications:

• 2-3 years of experience in skilled nursing, hospital discharge planning, case management, or healthcare marketing.
• Prior admissions or liaison experience preferred.
• Knowledge of Medicare, Medicaid, managed care, and PDPM preferred.
• Knowledge of EMR systems and Microsoft Office
• Frequent travel to hospitals and community partners
